Put test flavor in DISABLED issue title #5194

A test that's failing on rocm only is very different than one that is failing on plain is very different from one failing on dynamo config, but they're all jumbled together in the title of disabled issue, e.g., pytorch/pytorch#125924 vs pytorch/pytorch#125926

@ezyang Are you wanting to list all the platforms in the title itself, since there can be more than one?

So, most specifically, I want to know:

  1. Is this a ROCM only issue?
  2. Is this a Windows only issue?
  3. Is this a Dynamo/Inductor special config only issue?
