It looks like the mission indicators (is there a proper name for them?) are displaying per planet you have to land on, not per system you have to visit. This means that if a single mission requires you to land on two planets within a system then two mission indicators display on that system on the map.
I've always thought of those indicators as per-system, not per planet, so this is confusing to me. Should be one per system per mission. Maybe this is intended behaviour though, in which case I'll close this.

Expected Behavior
Only show one mission indicator per system per mission. The mission info should tell you if you need to visit multiple planets.
